Michgian's Nursing Facility Transition Initiative recently joined efforts with the Michigan Medicaid
Waiver Agents to expand usage of the Home and Community Based/Elderly and Disabled Waiver
service care planning and service delivery system.  
This collaborative effort will greatly enhance
the delivery of transition services to people in nursing facilities who wish to return to the
community
.

Overview

NFTI is a program offered by the Michigan Department of Community Health funded by the
Centers for Medicare and Medicaid under a Real Choice Systems Change Grant.  Its goal is to
change existing systems to transition and divert people, wishing to remain in the community,  from
nursing home stays.  To date, NFTI has assisted over 200 elderly and disabled people remain in the
community.
